Scheduling an appointment is easy and can be done by calling our office, visiting our office in person, or online.
If you wish to schedule an appointment over the phone, you can do so by calling us during normal business hours at 1 (636) 916-0050.

We use Microsoft Bookings for online scheduling. To make your appointment online, you can visit the specific service page under the Services menu and click the "Schedule An Appointment Now" button, or click here to browse all of the services that can be scheduled.
Before you make an appointment, you should make sure you have all of the following information available:
- Your name, or the name of the donor if you are scheduling for someone else
- Your telephone number
- The date and time that you prefer
- The type of test being performed
- If any legal order of protections or restraining orders are in place between you and any other donors, we will need to know the names of the individuals the orders pertain to so that we can attempt to accommodate appointment times
If you are scheduling an appointment for a minor, you may also need to provide:
- Name of the minor
- Age of the minor
- If a parent or guardian will not be accompanying the minor to the testing site, prior written authorization from a verified parent or guardian must be in place
Some tests may require additional information to schedule an appointment. We will let you know when scheduling what that information is.
